Terms & Conditions

These Terms and Conditions ("Terms") govern your use of services provided by KDM Derma Therapy LLC ("we", "us", "our", or "KDM Derma Therapy") at 1611 East Centerton Boulevard, Centerton, Arkansas 72719. By scheduling, attending, or purchasing services from KDM Derma Therapy, you agree to these Terms.

1. Booking and Appointments

All appointments are scheduled through our authorized booking platform at kdmdt.glossgenius.com or by phone at 479-403-8678. A deposit may be required at the time of booking to secure your reservation. Deposits are applied toward your service total at the time of your appointment.

1.1 Confirmation and Reminders

You will receive booking confirmation by email and reminder messages by SMS or email prior to your appointment. It is your responsibility to keep your contact information current. We are not liable for missed appointments resulting from outdated contact details.

1.2 Late Arrivals

Please arrive at least 5 minutes before your scheduled time. Arrivals more than 15 minutes late may result in a shortened service, rescheduling, or forfeiture of the deposit at our discretion.

2. Cancellation and Rescheduling

• 24 hours or more in advance. Reschedule freely. Deposits transfer to the new appointment.
• Less than 24 hours but more than 4 hours. Reschedule permitted once. Deposit may be partially retained.
• Less than 4 hours or no-show. Full deposit forfeited. Repeat no-shows may require pre-payment for future bookings.

3. Payment

Payments are accepted via credit card, debit card, and Square invoicing. All prices are listed in U.S. dollars and are subject to change without notice. The price listed at the time of booking is the price you pay for that service.

3.1 Gratuities

Gratuities are appreciated but never required.

4. Refunds

Services rendered are non-refundable. If you are unsatisfied with a service, please contact us within 7 days at contact@kdmdermatherapy.com. We will work with you on remedy options at our discretion, which may include a complimentary corrective treatment.

Retail products (where offered) may be returned unopened within 14 days of purchase for a full refund.

5. Health, Safety, and Treatment Consent

Many of our services involve the application of active ingredients, mechanical exfoliation, or energy-based treatments. Your safety depends on accurate information shared with your practitioner.

5.1 Required Disclosures

Prior to receiving any service, you agree to disclose:

• Known allergies to skincare ingredients, latex, fragrance, or topical products
• Current medications, particularly retinoids, blood thinners, and isotretinoin (Accutane)
• Pregnancy or breastfeeding status
• Recent sun exposure or tanning bed use within the past 14 days
• Skin conditions including but not limited to active acne, rosacea, eczema, herpes simplex, recent surgeries, or cancer treatments
• Use of accutane within the past 12 months

5.2 Treatment-Specific Consent

The following services require a signed consent form before treatment begins. Forms are reviewed in person at your appointment:

• Chemical peels and acid treatments
• Microneedling and collagen induction therapy
• Jet plasma facials
• Dermaplaning
• Eyelash extension removal services

5.3 Assumption of Risk

All aesthetic treatments carry some risk. Possible side effects include but are not limited to redness, swelling, sensitivity, peeling, breakouts, post-inflammatory hyperpigmentation (particularly relevant in skin of color), and rarely scarring or allergic reactions. You acknowledge these risks before receiving service.

6. Suitability and Right to Refuse

We reserve the right to refuse, modify, or discontinue any service if we determine, in our professional judgment, that the service is not in the best interest of your skin, scalp, or overall health. In such cases, alternative treatments will be recommended.

7. Photography and Likeness

We may, with your written permission, take before-and-after photographs of your treatment for clinical documentation or marketing purposes. You may opt out at any time. Photos will not be shared publicly without explicit consent.

8. Minors

Clients under the age of 18 must be accompanied by a parent or legal guardian who must provide written consent. Certain advanced treatments (microneedling, chemical peels, jet plasma) are not available to clients under 18.

9. Gift Cards and Promotional Offers

Gift cards are non-refundable and may not be redeemed for cash. Promotional offers cannot be combined with other discounts unless explicitly stated. Gift cards do not expire.

10. Health and Wellness Disclaimer

KDM Derma Therapy provides cosmetic and wellness services. We are not a medical practice and do not diagnose, treat, cure, or prevent any disease. If you have a medical condition, please consult with a licensed physician before scheduling treatments.

11. Limitation of Liability

To the fullest extent permitted by law, KDM Derma Therapy LLC, its owner, employees, and affiliates shall not be liable for any indirect, incidental, consequential, or punitive damages arising from your use of our services. Our total liability for any claim shall not exceed the amount you paid for the service that gave rise to the claim.

12. Glow Social and Private Event Reservations

Private event reservations (Diva's Den, Mommy and Me Derma Cleanse Class, Holistic Harmony Oasis) require a non-refundable booking deposit. Final payment is due 48 hours before the reserved date. Group size, service menu, and start time will be agreed upon in writing at the time of booking.

13. Changes to These Terms

We may update these Terms from time to time. The current version is always posted at kdmdermatherapy.com/terms.html with the effective date listed at the top.

14. Governing Law

These Terms are governed by the laws of the State of Arkansas. Any disputes shall be resolved in the state or federal courts located in Benton County, Arkansas.
